Please fill out the form below to refer a child for the early intervention program.

Make a referral to Kids On The Move if you are concerned that your child is not reaching developmental milestones.

Requirements

To apply for Early Intervention services through Kids On The Move:

  • YOU MUST reside in the Alpine School District boundaries (Orem, Vineyard, Lindon, Pleasant Grove, American Fork, Highland, Cedar Hills, Alpine, Lehi, Saratoga Springs, Eagle Mountain, some parts of Draper)
  • It must be 45 days before your child turns 3 years of age. If your child is over this age, please call the Alpine School District Assessment Center at 801.717.4835 for a free assessment.

You should be contacted by Kids On The Move in FIVE working days. If you have not been contacted in that time, please call us at 801.221.9930.
